Helping Minnesota Families Stay Healthy
Child & Teen Checkups (C&TCs) are comprehensive checkups for MA or MNCare eligible children and adolescents ages birth to 21 years of age. Children and teens who have regular checkups have the best chance to stay healthy, do well in school, and thrive.
The NE Regional Child & Teen Checkups Partnership is a collaborative of county C&TC Coordinators, Providers, Health Plans, and Minnesota Department of Health representatives. The Partnership provides:
- Outreach to families about the benefits of C&TCs and the additional screenings and guidance provided at a C&TC
- Information about Health Plan incentive programs
- Information about obtaining appointments, transportation, and interpretation services
- Information and tools to assist Health Care Providers in providing the best possible service to the families of Northeast Minnesota
Large Providers List Helps Families Find Nearby Healthcare
The NE Regional C&TC Partnership has compiled a listing of health care and dental care providers that accept Medical Assistance (MA) and/or MinnesotaCare (MNCare) clients. Providers are listed by county. However, you may access providers across county lines that may be closer to your home. If you need assistance getting an appointment with any of these providers, you may contact your health plan or your county's Child & Teen Checkups Coordinator. Your health plan and/or county coordinator may also assist you with transportation or interpreter needs.
Family Health Documentation and Forms
Providers, are you looking for forms to assist you in providing complete Child & Teen Checkups every time? The NE Partnership has provided easy access to forms, guidance, and information on C&TC on the provider page. Also includes forms created within the NE Region to assist receptionists, nurses, doctors and FNPs, and billing staff in communicating C&TC information. Check it out!

The Minnesota Student Survey (MSS) is administered statewide every 3 years with the latest survey taking place in 2010. The survey includes students in grades 6, 9 and 12.
